Reporting Student Injuries

Procedure in the Event of a Student Injury

• Students must immediately report the injury to the teacher.
• If necessary, the teacher will ensure that first aid is provided and that the injured student receives medical treatment.
• The teacher is responsible for recording every injury in the Student Injury Logbook.

In case the student receives medical examination or treatment:

• During the medical examination/treatment, the student must ask the doctor to complete the “Pain Assessment Report” in case the injury is to be reported to the insurance company (ČPP).
• The student must also complete a Statutory Declaration for insurance purposes.
• The student sends the original copies of the following documents to hlaseni.urazu@uhk.cz:
• Pain Assessment Report – „Přehled bolesti“ (The form must be completed in the Czech language)
• Statutory Declaration – „Čestné prohlášení“ (The declaration must be completed in the Czech language)
• records of all medical examinations and treatment related to the injury
• all receipts and documents confirming payments related to the treatment
• confirmation of study status (valid at the time of the injury) 

The confirmation of study status can be found in STAG → My Study → Course of Study, timetable → Student Status cerfiticate

After being reviewed by the Occupational Health and Safety (OHS) specialist, these documents will be forwarded to the insurance company for assessment and claim processing.
