Applications
Use the General Request Form unless a specific form is prescribed for a particular procedure (see the Applications subpage).
You may submit your request in person at the Study Department during office hours, by post to the correspondence address, via the student’s data mailbox (for natural persons) registered in the student’s personal data. Selected types of requests may be submitted through IS/STAG: My Study – Student Requests.
If the request is subject to a fee according to Dean’s Directive No. 3/2013, please pay the fee to the following account:

Attach proof of payment to your request (confirmation of the transaction, or clearly state the account number and bank code from which the fee was paid). The fee must be paid before submitting the request to the Study Department.
